Description Completed self-assessment questionnaires of the Whitehall Study comprising of questions relating to personal identity, employment status, smoking habits and history, respiratory conditions, cardiovascular health, medical history and clinical assessment. The original questionnaires were submitted on the same day of the clinical examination and date from 1967-1970. They can also include additional information submitted by the volunteer, mainly regarding medical history, and also any correspondence with the volunteer's doctor as a result of the medical inspection. Annotation on the front page of the questionnaire indicate whether the volunteer had been selected for control studies (smoking cessation, borderline diabetes), with a red sticker indicating they are part of a control group and yellow indicating being part of the treatment group.

The questionnaires are arranged in two separate series: the first series contain collected questionnaires with the second set of questionnaires including photocopies of death certificates of the volunteers. Each series is arranged in a numerical order based on the assigned survey number beginning from 05000 to 24999.
